Open Incident
Open Incident allows users to manage incident reports. You can record clients, staff members, witnesses, others involved in the incident, an incident’s time, place, and severity level. You also have the option to edit incident reports if you need to adjust information originally submitted.
Note: To edit a submitted incident, the status of the incident must be open and not closed. Below are detailed steps for completing an Open Incident Form.
Basic Incident Information
- Go to: Avatar PM/Avatar MSO > System Maintenance > Open Incident
- To open a new incident, in the Select Incident screen, enter the incident name or ID# and click Search.
- If there is an existing incident, it will appear in the results list. Click the result, and then click Select.
- If no existing incident is found, click New Incident. The Open Incident form displays.
- In the Brief Incident Description field, enter a title description for the incident.
- The Status field displays the incident status.
- Incident status can be changed in the Close Incident form.
- In the Date Incident Occurred field, enter the incident date.
- In the Time Incident Occurred field, enter the incident time.
- In the Date Incident Reported field, enter the incident report date.
- In the Time Incident Reported field, enter the incident report time.
- Enter the Incident Description.
Advanced Incident Information
- In the Program Of Incident field, select the client's treatment program.
- Treatment programs are set up in the parent application.
- In the Shift When Incident Occurred field, select the shift.
- In the Incident Classification field, select the incident classification.
- In the Incident Severity field, select the severity.
- The Treatment Setting field displays the treatment setting associated with the selected program of incident.
- In the Contributing Factors field, select the factors.
- In the Type Of Incident field, select the incident type.
- In the Incident Location field, select the location.
- In the Was Incident Self-Inflicted Or Physical Assault field, identify the incident as Self-Inflicted, Accident, Physical Assault, or Unknown.
- In the Incident Location/Details field, enter details as necessary.
Incident Record Information
- The Person Completing Incident Form field displays the user who opened this form. Edit as necessary.
- In the Date Completed field, enter the incident report completion date.
- In the Time Completed field, enter the incident report completion time.
- In the Facility Location field, select the facility's location.
- Click Submit.
Note: To edit a closed incident, the incident's status must be changed to Open in the Close Incident form.
