Close Incident

Close Incident allows you to either close an incident or open a closed incident. You can also use this form to indicate corrective actions that have taken place as well as the person that the incident was reported to.
Note: For a closed incident to be edited on the Open Incident form, it must be changed to open in the Close Incident form. 


  1. Go to: Avatar PM/Avatar MSO > System Maintenance > Close Incident
  2. In the Select Incident screen, search for the incident name or ID#, and select the incident from the list of results. The Close Incident form displays.
  3. In the Incident Status field:
    • Select Open to open a closed incident.
    • Select Closed to close an open incident.
  4. The Incident Closed By field displays the user who closed or is closing the incident. Edit as necessary.
  5. In the Date Incident Closed field, enter the incident closed date.
  6. In the Significant Incident field, select Yes if the incident was significant.
  7. In the Corrective Actions field, select the corrective action.
  8. In the Incident Reported To field, select the person the incident was reported to.
  9. Select Submit.
