Client Charge Input With Diagnosis Entry - Cal-PM
Enter services (charges) for a client.
- Entering a service posts the service fee to the client ledger, and adds the services to the client’s treatment history.
- The Cost Of Service field can be edited in the Client Charge Input (Charge Fee Access), and Client Charge Input (Charge Fee Access and Diagnosis Entry) form.
- 'Medical Diagnosis' fields are available in this form, and in the Client Charge Input (Charge Fee Access and Medical Diagnosis) form.
- A managed care authorization must be setup for the client in the Managed Care Authorizations form.

Go to: Avatar PM > Services > Ancillary/Ambulatory Services - In the Date Of Service field, enter the service date.
- In the Client ID field, enter the client name, select the Search button, and the corresponding entry.
- In the Episode Number field, select the client episode.
- In the Program field, select the client program.
- In the Service Code field, enter the service code, select the Search button, and the corresponding entry.
Note: The service must be required by a client, or a client and practitioner (Service Required By field, Service Codes form). - In the Practitioner field, enter the practitioner name, select the Search button, and the corresponding entry.
- Select Display Managed Care Authorization Data.
Note: The Managed Care Authorization Data screen displays managed care authorizations for the client, service code, and date of service. - In the Duration (Minutes) field, enter the service duration.
- In the Location field, select the service location.
- In the Co-Practitioner field, enter the practitioner name, select the Search button, and the corresponding entry.
- In the Co-Practitioner Duration (Minutes) field, enter the service duration.
- In the Co-Practitioner 2 field, enter the co staff member, click the Search button, and select the corresponding entry.
- In the Co-Practitioner 2 Duration (Minutes) field, enter the service duration.
- The Cost of Service displays the service fee, setup in the Service Codes and Service Fee Maintenance forms.
- In the Active Medical Diagnosis field, select the medical diagnosis.
Note: This list contains medical diagnoses associated with the client (Medical Diagnosis form). - In the Diagnosis fields, enter the ICD-10 code, select the Search button, and the corresponding entry.
- In the Evidence-Based Practices / Service Strategies (CSI) field, select the service strategies. This field provides additional service information.
- In the Additional Service Information field, select the applicable codes.
- In the Psychotherapy Add-On Duration field, enter the add-on duration in minutes.
- In the Add-On Notes field, enter any additional information related to the add-on.
- Select Submit.
