Location - Dictionary Value

Location is set up in the Dictionary Update form (Client file - data element 10006).
Enter the various locations where services are performed.

This dictionary has several extended data elements - Extended Dictionary Value (Single Dictionary) field:

• 2600 - Rendering/Billing Lab's CLIA Number
  Enter the lab's CLIA number.
• 2601 - Outsourced Lab's CLIA Number
  Enter the lab's CLIA number.
• 578 - Place of Service (HCFA 24-B)
  Select the place of service to populate for paper billing.
• 579 - Place of Service (837 Professional)
  Select the place of service to populate for electronic billing.
• 7000 - Address - Street
• 7001 - Address - ZIP Code
• 7002 - Address - City
• 7003 - Address - State
• 70308 - Place of Service (CSI)
  Select the place of service to populate.
