Treatment Plan Input (MH) - (Workflow)

Create a mental health treatment plan for a client.

  1. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click Ok.
    If the client has multiple episodes, select the client's episode, click Ok.
    The Plan ID field displays the plan ID that increases with each new treatment plan and cannot be edited.
  2. In the Plan Start Date field, enter the date.
  3. If appropriate, in the Reviewing Staff Member field, select the staff member.
    Staff members are assigned to be notified in the Notification Users form.
  4. In the Treatment Plan Status field, select the status.
    If the user is on a required notification list, Final is disabled.
    If the user is not on a required notification list, Pending Approval is disabled.
  5. Enter other information as appropriate.
  6. Click Diagnosis Display to view the client's diagnosis information. The report displays the client diagnosis entered in the Diagnosis form (Avatar PM).
  7. Once finished, click Submit.
