Enter Group Default Note (Workflow)

Create a progress note for each member of a group, when a group service (associated with workflow functionality) is filed.

  • Information entered for a group service in this form can be edited for an individual client. Use the Edit Group Note for Individual (Workflow) form to enter progress notes for individual clients who were part of the group service.
  • If the service requires a progress note the progress note can be opened from the To-Do item in the My To Do List frame. Progress notes can be entered in this form, or they can be entered for individual clients through the To-Do list.

Prerequisites:

  • The clinician must have rendered a group service for the date entered in the Date Of Service field (Group Appointment Scheduling form, myAvatar PM).
  • The clinician's schedule must be verified in myAvatar PM (Verify Staff Member Appointments form), and posted (Post Staff Member Schedule form). Or, through the Post Staff Activity Log form which verifies and posts the appointment.
  • Functionality can be added to this form through Site Specific Section Modeling.
  1. In the Scheduled Clinician field, enter the clinician's name, click Process Search. Click Ok.
  2. In the Date Of Service field, enter the date.
    Group services entered in the Group Appointment Scheduling form (Avatar PM) are available.
  3. In the Service(s) field select the group service performed on the date.
  4. In the Note Type field, select the progress note type.
  5. In the Note field enter associated progress note text.
  6. In the User To Send Scratch Note To-Do Item field, select the user.
    This user will receive a To-Do item to co-sign a progress note for each group member.
    The User List Management and Notification Users forms define available users.
  7. If appropriate, select the user to co-sign the To-Do item.
    Depending on the note type selected this field may be enabled or required.
  8. Click Submit.
