Enter Vital Signs Screen

Enter vital signs.

If values for a vital sign are outside the normal range (CWS Vital Signs Setup), they will display in red.

Buttons

  • New Row - create a new row.
  • Delete Row - delete a row.
  • Save - submit vital signs information.
  • Close/Cancel - close the screen without saving changes.
  • Copy/Paste Row - select a row to copy or paste.
  • Copy Cell - select a cell to copy.
  • Paste Cell - select a cell to paste.

Fields

  • Date - enter the date in the format MM/DD/YYYY. Or, click the down arrow to display a calendar. Double-click a date to enter.
  • Time - enter the time in the format HH:MM. Enter PM if appropriate.
  • Refused Vitals - select Y or N. If Yes is selected, the remaining fields in this screen are disabled.
  • Temp (F) - enter the client's temperature in either Fahrenheit or Celsius.
  • Temp (C) - enter the client's temperature in Celsius.
  • Heart Rate (bpm) - enter the client's heart rate in beats per minute.
  • Respiration Rate (bpm) - enter the client's respiration rate.
  • Oxygen Saturation (%) - enter the percent of the client's oxygen saturation.
  • Blood Pressure Systolic (mmHg) - enter the client's systolic blood pressure.
  • Blood Pressure Diastolic (mmHg) - enter the client's diastolic blood pressure.
  • Position - select the client's position.
  • Enter the client's height in either feet and inches, total inches, or centimeters.
    The client's height is converted in the other height fields.
    • Height (ft in) - enter the client's height, in the format 'feet, *space*, inches'. For example, '5 11'.
    • Height (in) - enter the client's height in inches.
    • Height (cm) - enter the client's height in centimeters.
  • Enter enter the client's weight in either pounds or kilograms.
    The client's weight is converted in the other weight field.
    • Weight (lbs) - enter the client's weight in pounds.
    • Weight (kgs) - enter the client's weight in kilograms.
  • BMI - the client's BMI is calculated when height and weight are entered.
  • Pain Scale - select the client's pain scale.
    • 0 – no pain
    • 10 – greatest pain
  • Blood Glucose (mg/dl) - enter the client's glucose level in milligrams per deciliter.
  • Comments - enter comments associated with the vital signs.
