Define forms as assessments

Prerequisites

  • Forms must be defined as assessments in the Flag Assessment Forms form. Problem codes map to dictionary fields of Admission and user modeled forms that have been defined as assessment forms in Flag Assessment Forms.
  • Filing a mapped form will update the problem list to the mapping.

Problem List

This enables the mapping of dictionaries to SNOMED problems in the Problem List form.

  1. Go to Avatar CWS > CWS Utilities
  2. In the Map Code for Use In field, select Problem List. This allows you to map problems to another form.
  3. In the Form to Map field, select the assessment form that this mapping is from.
    • If needed, select the specific section of the form in the Section field. This is used to filter the selection of dictionary fields.
  4. The Map To field defaults to Problem for problem list mappings.
  5. The Type of Mapping field defaults to Dictionary.
  6. In the Assessment Field field, select the field to create the mapping for.
  7. In the Assessment Response (Dictionary) field, select the dictionary to map.
  8. In the Associated Code field, enter the SNOMED or problem code, and select.
    Note: When the associated problem​ is mapped to an assessment field and dictionary, entering this information in the associated assessment (Form to Map field), will populate the code in this field to the client's problem list.​
  9. In the Default Problem Status Code field, select the default problem status.
  10. The Default Problem Active Status field displays the extended dictionary value for the problem status selected in the Default Problem Status Code field.
  11. In the Default Problem Date From field, select the assessment date field.
    If no selection is made, the current date will be used.
  12. Click Save Mapping to save file your mapping information.
  13. Click Remove Mapping to delete mappings created for this form.
  14. Click Display Mappings for Selected Form to generate a report that lists mappings for the selected form.
Progress Notes
  1. Go to: Avatar CWS > CWS Utilities
  2. In the Map Code For Use In field, select Progress Notes.
  3. In the Form to Map field, select the assessment form that this mapping is from.
  4. The Map To field defaults to Product Custom Logic.
  5. In the Type of Mapping field, select the field type.
  6. In the Assessment field, select the field to create the mapping for.
  7. In the Product Custom Logic Description field, enter a description that can be used in the Site Specific Section Modeling form. This is enabled when Progress Notes is selected in the Map Code for Use In field. When a mapping is saved, a new entry will be created in the Product Custom Logic section of the Site Specific Section Modeling form.
    Note: The Product Custom Logic definition is tied to an assessment field. When a mapping is removed, the definition is also removed from all assessment fields and response combinations.
  8. In the Product Custom Logic Start Date and Product Custom Logic End Date fields, select the date to use for Progress Notes mapping types where the Assessment field selected is in a multiple-iteration table.
    • This field is used when mapping multiple-iteration table fields to populate single-select or multiple-select dictionaries on the progress notes forms with only active values.
    • When this field is used, a data dictionary containing all active values is populated instead of a single default value for the mapped field on the progress notes form. Active values for the dictionary are determined by comparing the Date of Service (if populated) and current date (if Date of Service isn't populated) to the date range specified with these fields. 
    • If no values are entered for the Product Custom Logic Start Date field and the Product Custom Logic End Date field, the current data value of the Assessment field populates the mapped field on the progress note form.
  9. Click Save Mapping to save file your mapping information.
  10. Click Remove Mapping to delete mappings created for this form.
  11. Click Display Mappings for Selected Form to generate a report that lists mappings for the selected form.

Note: For each new note created, this data will come forward from the last row submitted from the "source" assessment.

Note: When Draft/Final functionality is enabled, notes saved in Draft will retain old values when the draft note is edited.

Treatment Plan
  1. Go to: Avatar CWS > CWS Utilities
  2. In the Map Code for Use In field, select Treatment Plan.
  3. In the Form to Map field, select the assessment form that this mapping is from.
    • If needed, select the specific section of the form in the Section field. This is used to filter the selection of dictionary fields.
  4. In the Map To field, select whether to map to the Strengths, Weaknesses, or Discharge Plan information on the Treatment Plan form.
    • When the associated problem​ is mapped to an assessment field, entering this information in the associated assessment will populate the discharge plan, strength, or weakness in this field to the client's treatment plan.​
    • This allows mappings from user-defined fields to populate the associated fields in the Treatment Plan form.
  5. In the Assessment field, select the field to create the mapping for.
  6. Click Save Mapping to save file your mapping information.
  7. Click Remove Mapping to delete mappings created for this form.
  8. Click Display Mappings for Selected Form to generate a report that lists mappings for the selected form.
