Problem List

This assessment is used to document client problems like to screen drug to disease contraindications, associate a progress note to a problem, and associate an order to a problem. It uses the SNOMED Core subset (Systematized Nomenclature of Human and Veterinary Medicine Clinical Terms). This is a subset of the complete SNOMED CT terminology. (SNOMED problem descriptions are loaded into myAvatar when this form is installed.)

Problems are entered in this form, and in the Treatment Plan form. Once a problem is entered here it is added to the Treatment Plan form. Problems entered in the Treatment Plan form populate this form.

Prerequisite: Ensure the appropriate updates for ICD-10 Problem List - Diagnosis Content on Demand have been installed.


  1. Go to: Avatar CWS > Assessments > Product Assessments > Problem List
  2. In the Problems To Record field:
    • Select Yes to enable the Enter Problems field. (If a client has existing problems, this field is disabled and set to Yes.)
    • Select No if the client does not have problem information to record. This will save a single entry to the client's problem list, noting that they have been inquired about problems.
  3. Click View/Enter Problems to display the Problem List screen.
  4. In the Status field, select the problem status.
    Note: Problem status is used to filter the display of the Client Problem report.
  5. Click Problem List Report to generate the Client Problem report that details all problems in myAvatar CWS.
    Result: The report displays problems associated with the selected problem status (Status field).
