ARRA Measures Information Source

The following details where ARRA Measures information is entered in myAvatar .

Active Medication Allergy List
  • Allergies And Hypersensitivities form.
Active Medication List
  • Client Profile / Physicians Orders form, myAvatar Order Entry.
  • Order must be active. The order must not be discontinued, or a hold order.
  • If the order is preauthorized, it must be activated (Avatar Order Entry, Activate Pre-Authorized Orders form).
CPOE for Med Orders
  • Avatar Order Entry - Client Profile / Physicians Orders form.
  • Order must not be active.
Lab Test Results
  • Avatar Results Table module - Results Entry form.
  • An entry must be filed in the Observation Value field (Results Details section).
  • One or more of the following values can be entered in this field: POSITIVE, Positive, positive, POS, Pos, pos, NEGATIVE, Negative, negative, NEG, Neg, neg
Medication Reconciliation
  • In order to be listed in the denominator: The client must be transferred to a practitioner associated with ARRA reporting (Practitioner Enrollment form, myAvatar PM).
  • The numerator lists the number of client transitions where medication reconciliation was performed (Medication Reconciliation form).
Patient Clinical Summaries of Visit
  • A service associated with ARRA reporting must be rendered.
Patient Electronic Copy of EHR
  • Disclosure Management form (myAvatar PM).
  • In order to be listed in the numerator: The disclosure date must be filled in (Disclosure Date field, Disclosure section).
  • In order to be listed in the denominator: The request date (Request Date field, Request section), and the disclosure date (Disclosure Date field, Disclosure section), must be within three business dates of each other.
Patient Reminders
  • Not associated myAvatar form.
  • In order to be listed in the denominator: The client must be five years old or younger or 65 years old or older.
Patient Specific Education
  • Client/Caregiver Education form.
Patient Timely Electronic Access To EHR
  • Client must be associated with an myAvatar ConsumerConnect user account.
Problem List
  • Problem List form.
Record Demographics
  • Client demographics must be entered in any of the following forms of myAvatar PM: Pre Admit, Admission, Update Client Data
  • In order to be listed in the numerator, the following information must be recorded: Gender, Race, Ethnicity, Date of Birth, Preferred Language
Record Smoking Status
  • Admission form (Other Client Data section), and Update Client Data form - myAvatar PM
  • In order to be listed in the numerator, the client must be 13 years old or older.
Record Vital Signs
  • CWS Vital Signs form.
  • In order to be listed in the denominator, the client must be two years old or older.
  • In order to be listed in the numerator, the client must have at least on entry for blood pressure, or BMI.
Summary of Care at Transitions of Care
  • The client must be transferred from one clinical setting (inpatient, outpatient, facility) to another
  • In order to be listed in the denominator the client must be have a transfer of care or referral to an external hospital.
Transmit Prescriptions Electronically
  • Client Profile/Physicians Orders form (Avatar Order Entry
