CWS Vital Signs

If myAvatar Order Entry is installed, the client height and weight information entered in this form is entered in myAvatar Order Entry. System Administrators can add functionality can to this form using Site Specific Section Modeling.

Prerequisites:

  1. Go to: Avatar CWS > Other Chart Entry
  2. In the Client Lookup field, enter the client name and select from the list.
  3. In the Grid Start Date and Grid End Date fields, enter the date range to display in the Vital Signs grid.
  4. Select Enter Vital Signs to display the Enter Vital Signs screen.
  5. In the Vital Sign For Graph field, select the type of vital sign information to display in the graph.
  6. Select View Graph.
    Note:  On the CWS Vital Signs Graph report, individual graphs display the client's vital sign information over the date range.
  7. In the Vital Sign(s) for Report field:
    • Select the vital signs to display in the Vital Signs Report (View Report field).
    • Select All to display all vital signs in the report.
  8. Enter the Start Date, Start Time, End Date and End Time in the correct fields.
  9. Select View Report.
    Note: The CWS Vital Signs Setup report displays vital sign ranges entered in this form.
  10. Select Launch Growth Chart.
    Note: The Growth Chart report displays the CDC body measurements for children.
