Customize Wellness Section

Selecting a Wellness Item populates medication information.

  1. In the Wellness Item field, select the wellness Item.
  2. In the Declined field:
  • Select Yes if the wellness item was declined. (A client alert will not be sent.)
  • Select No if the wellness item was administered.
  1. If the wellness item was declined, in the Declined by field, select the person who declined the wellness item.
  2. In the Age field, enter the client's age.
  3. In the Yr/Mo field, select the unit of time for the Age field.
  4. In the Gender field, select the client's gender.
  5. In the Interval # field, enter the number of months, years, or days the wellness item is due.
  6. In the Interval Units field, enter the unit of time for the Interval field.
    Note: Select N/A if the wellness item is not associated with time.
  7. In the Recurring field, select Yes if the wellness item is recurring.
  8. In the Start Date field, enter the customization start date.
  9. In the Customization Reason field, select the reason the health maintenance guideline was customized.
  10. In the Comments field, enter the reason for customization.
