Transfer client to another program - PM

Transfer a client from one program to another in an episode.

  • Transfers can only occur between the same type of program. For example, from outpatient to outpatient, or inpatient to inpatient.

  1. Go to Avatar PM > Client Management > Episode Management > Program Transfer
  2. In the Select Client screen, enter the client name or ID, and select. Choose Select.
  3. The Program Transferred From field shows the current program assignment.
  4. The Current Treatment Setting field shows the program treatment setting.
  5. For Program, select the treatment program.
    • The choices in this field are limited to only those programs that have a matching sub-system code facility ID as the program from which the client is being transferred.
    • This is only applicable for those systems that are configured to allow a facility ID to be defined at the sub-system code level.
    • The Allow Admission To File/Edit Pre-Admits registry setting can affect the Program field.
  6. The Treatment Setting and Treatment Service fields display information associated with the program.
  7. For the Date Of Transfer, enter the date the client is assigned to the new program.
  8. For the Time Of Transfer, enter the program transfer time.
  9. Select the Unit.
  10. Select the Room.
  11. Select the Bed.
  12. The Licensed/Unlicensed field shows the bed license status.
  13. The Time Of Bed Assignment field displays the time entered in the Time Of Transfer field.
  14. Select the Room and Board Billing Code.
  15. Select the Admission Charge Code.
  16. Select the Daily Charge Code.
  17. For partial hospitalization, day treatment programs:
    • For Partial Hospitalization Days, select the days of the week the client will be treated in the hospital.
    • For Partial Hospitalization Hours, enter the session hours.
    • Select the Partial Hospitalization Billing Code.
    • If applicable, for the Partial Hospital Billing Code 2, select a second billing code.
  18. For the Team Assignment, assign the client to a selected team.
  19. Select Submit.
