Health Status Evaluation - Milestones/Final window

Note: To access this window, you need to have the appropriate privilege granted by the Netsmart Homecare administrator.

Final milestones create Health Status Evaluation, that is the scoring that is available when the patient is discharged and the evaluation is documented.

With the Health Status Evaluation window, you can do the following:

  • Define the maximum value for the health status evaluation index scoring scale.
  • Add, edit, and delete the health status evaluation of patient's goals. For more information, see Manage Health Status Evaluation Values (Milestones). Note: You can enter the health status evaluation values that you created in Patient>Clinical>Health Status Evaluation only for patients who are discharged.
  • Define the index for each health status evaluation value.
  • Determine whether you want to include the defined index in the calculation of the Overall Achievement Index that can be previewed in the Goals/Expected Health Status Evaluation section of the patient's Discharge Summary report.

Note: If the health status evaluation index equals zero, do not include it in the calculation because it may cause inaccurate results.

Click  to define the maximum value for the Health Status Evaluation index scoring scale.

Code

Enter the code for this health status evaluation, up to two alphanumeric characters.

Description

Enter the description for this health status evaluation, up to 60 alphanumeric characters.

Index

Enter the index for the value (within the Health Status Evaluation index scoring scale).

Do Not Include in Calculations

Select this check box to exclude defined index from the Overall Achievement Index calculation.
