Visit Frequency report – Define tab

Use the Define tab to specify the information you want to include in the report. Specify the conditions for generating the report.

This tab contains the following sections you need to complete. Select a section from the list to go to that section.

 

Date range

Select the time period the report should be generated for:

Option Description

Begin Date

Specify the beginning date of the reporting period.

End Date

Specify the end date of the reporting period.

Selection criteria

Not available from the Patient component

To select one or multiple patients you want to generate the report for, use the following options:

Option Description

Team

Select to include in the report only patients of a certain team.

Specific Patients

Select to run the report for specific patients.

Medicare Patients Only

Select to include only Medicare patients.

Include in report

To include specific data in the report, select the following check boxes:

Option Description

Include In Process Visits

Select to consider all the unverified visits actual and include them in the report. If you select this check box, the Include In Process Visits section becomes available.

Show PRN Reason

Select to include reason for the PRN visit in the report.

Show Change Reason

Select to include the reasons for changing visit frequency in the report.

Visit Range option

To define the visit range for the report, select one of the following:

Use Both Low and High Number of Visits to Determine Exceptions

Select to include both high and low numbers of planned visits in the report.

Use High Number of Visits Only to Determine Exceptions

Select to include only high numbers of planned visits in the report.

Hour Range option

To define the hour range you want to use for the report, select one of the following:

Use Both Low and High Number of Hours to Determine Exceptions

Select to include both high and low hours of the planned visits.

Use High Number of Hours Only to Determine Exceptions

Select to include only high hours of the planned visits.

Include

To define the type of information related to frequency limit be shown in the report, select one of the following options:

Option Description

All Patients Over/Under/Met Frequency Limits

Select to include the patients who exceeded the planned limits, did not reach the planned limits, and who met planned limits. The report lists patients over planned visits first, then patients under planned visits, and then patients who met planned visits limit.

Patients Over Frequency Limits

Select to include all the patients who have scheduled and actual visits greater than planned visits and hours.

Exception: The patients whose scheduled and actual visits are more than the high number of visits per period plus PRN visits are not included.

Patients Under Frequency Limits

Select to include all the patients who have scheduled and actual visits less than planned visits and hours.

Exception: The patients whose scheduled and actual visits are less than the high number of hours per period plus PRN visits are not included.

Patients Over/Under Frequency Limits

Select to include all the patients who have scheduled and actual visits greater than planned visits and hours and the patients who have scheduled and actual visits less than the planned visits and hours. The report lists patients over planned visits first, and then patients under planned visits.

Include in process visit

Note: This section is active only if the Include "In Process" visits check box is selected in the Include In Report section.

Select one of the following options to use for the report:

Option Description

Treat "In Process" Visits as Actual

Select if you want the system to treat in process visits as actual visits.

Treat "In Process" Visits as Scheduled

Select if you want the system to treat in process visits as scheduled visits.

Show

To define the format of the report, click one of the following radio buttons:

Option Description

Both (Planned vs. Actual + Scheduled)

To print planned visits opposite to the actual visits and planned visits opposite to the scheduled visits.

Planned vs. Actual

To print planned visits opposite to the actual visits.

Planned vs. Scheduled

To print planned visits opposite to the scheduled visits.
