Convert free-text diagnoses and procedures (Host mode)

Path: Patient > General > Diagnoses

Preqrequisite: To convert free text, you need to have the appropriate privilege granted.

After synchronization from field mode, free-text diagnoses and procedures must be converted to the ICD-9, ICD-10, or both codes depending on the ICD-10 effective date and your agency's policy.

  1. In the Patient component, select a patient.
  2. Go to General > Diagnoses.
  3. Click the Medical Diagnoses, Procedures, or Inpatient Diagnoses tab. Free-text diagnoses and procedures are shown in the Free Text section.
  4. Click the ellipsis icon next to the free-text diagnosis or procedure. All free-text diagnoses or procedures are prefilled in the Select Diagnosis/Procedure Dialog.
  5. Double-click a diagnosis or procedure to add it to the Search field. If no search results appear, try rephrasing the description.
  6. After finding the corresponding diagnosis or procedure, double-click it to move it to the Selected section. Note: To delete a diagnosis or procedure from the Selected section, double-click it.
  7. Click OK.
    1. The selected diagnoses are added in the corresponding sections.
  8. Save your changes.

Important: If the diagnosis is identified as primary and the patient does not use diagnoses groups, make sure that this diagnosis is selected as primary in the Admissions & Status window. For patients with diagnoses groups, make sure that you set the sequence in a group because this cannot be done in field mode.
