Track immunization history

Use the Immunizations form in myEvolv to track the immunization history of a client or referral. This form is accessed from multiple modules:

  • Client > Client Information > Health Information > Immunizations
  • Referral > Agency Referral > Health Information > Immunizations
  • Program Referrals > Program Referral > Health Information > Immunizations
  • Referral Application > Application Data for Providers > Health Information > Immunizations
  1. Go to one of the above listed modules, then search for and select a client.
  2. In the toolbar, select New Event and choose an immunization.
  3. In the Immunizations dialog, complete any of the relevant information.
    Note: The Type displays automatically, based on the immunization event you selected. The Dose may also pre-populate.
  4. The Actual Date field is required. Click the calendar to select a date.
  5. Select Save.
