Track contact with a client or referral

Use the Contact Log and Contacts forms in myEvolv to document and track encounter information about the client pre-admission and post-admission. These forms are accessible from multiple modules:

  • Client > Client Information > Personal Information > Contact Log
  • Referral > Agency Referral > Referral Information > Contact Log
  • Referral > Agency Referral > Testing/Screening > Contacts
  • Program Referrals > Program Referrals > Testing/Screening > Contacts
  1. Go to one of the above listed modules, then search for and select a client or referral.
  2. In the toolbar, select Add New and choose an item (examples are court, family, and school).
  3. In the Contact form, enter the Actual Date/Time.
  4. Complete additional relevant information in any of the following sections:
    • No-Show Information - Select no show, reasons, and enter attempt to contact information. 
    • Encounter Information - Enter basic encounter information such as the activity type (for example, face-to-face) and if the client was involved. 
    • Completed Information - Enter the time and duration information.
    • Participating Staff Notes - Add any relevant notes.
  5. Select Save.
