COVID-19 Screening Tool - client

The myEvolv Client COVID-19 Screening Tool provides an instrument for screening and testing clients for the coronavirus. 

Prerequisites:

• A system administrator downloads the following documents to add a Client COVID-19 Screening Tool form in myEvolv:
  • Import a Form Design for the COVID-19 Screening Tool
  • myEvolv COVID-19 Screening Tool XML file
