COVID-19 Screening Tool - staff

In myEvolv the staff version of the COVID-19 screening tool has the same content as the client screening tool, but upon upload the form is saved to the People Requirements form family in order to link to the Requirements area under the staff profile.

Prerequisites:

• A system administrator downloads the following documents to add a Staff COVID-19 Screening Tool form in myEvolv:
  • Import a Form Design for the COVID-19 Screening Tool
  • myEvolv Staff COVID-19 Screening Tool XML file
