Assessments in Service Entry

In myEvolv use the Service Entry screen to enter, in one location, various types of services that use a variety of service entry forms, including assessments.  

These assessments can be:

  • Service events (service entry requires the user to specify the program and facility)
  • Non-service events
  1. Select the client in the My Clients widget.
  2. Navigate to the Assessments tab.
    Result: The assessment services previously entered as well as a Reassessment the system automatically scheduled are displayed.
  3. Select New Manual Event.
    Result: The list of New Manual Events includes non-service assessment events.
  • The Assessments tab in the My Client Information standalone form includes all assessment events as you see above. 
  • Your System Administrator can add tabs to My Client Information standalone form for various types of assessments.
