Add substance use history

In myEvolv document a client's substance use history.

  • Client > Client Information > Health Information > Substance Use
  • Referral > Agency Referral > Health Information > Substance Use
  • Program Referrals > Program Referral > Health Information > Substance Use
  • Referral Applications > Application Data for Providers > Health Information > Substance Use
  1. Go to one of the above listed modules, then search for and select a client.
  2. In the toolbar, select New Event then choose Substance Use.
  3. In the Substance Use dialog, complete any of the appropriate fields. The following fields are required:
    • Date Identified - Select the calendar to choose a date.
    • Priority - Select a priority.
    • Substance - Select an item from the Drug Library table. Examples of substances are cocaine, alcohol, and codeine.
  4. Select Save.
