Add Treatment History
Use the Treatment History form to track any treatment history for a referral or client that happened outside of the agency, such as previous substance abuse treatment.
- Required fields are Type and Procedure Description and, depending on the type of treatment you enter, additional fields are required. An example of Type is 'Out of Home Treatment', and for Procedure Description, 'Residential treatment'. No additional fields are required but it is recommended that you enter the dates, duration, and provider information.
- For clients, a placement disruption added in Client Information that has a Link Type of 'Location Known' appears in treatment history. Examples include Medical and Psychiatric Hospitalization and Legal Detention.
Menu paths
- Client > Client Information > Health Information > Treatment History
- Referral > Agency Referral > Health Information > Treatment History
- Program Referrals > Program Referral > Health Information > Treatment History
- Referral Application > Application Data for Providers > Health Information > Treatment History
- Go to one of the above listed modules, then search for and select a client.
- In the toolbar, select New Event and choose an event.
Note: The events displayed in the list may vary depending on your system configuration. The fields that are available to complete may vary depending on the event you select in this list. - Complete any of the relevant fields; the following fields are required:
- Type - The content is automatically populated based on the event you selected.
- Procedure Description - Enter a description.
- Select Save.
