Add Program Eligibility and Classifications
Use the Eligibilities/Classifications form in myEvolv to document eligibilities for the programs where a client, family case, or referral is enrolled.
Note: You can use the eligibilities/classifications as criteria for batch enrollment into a group.
Menu paths
- Client > Client Information > Personal Information > Eligibilities/Classifications
- Referral > Agency Referral > Referral Information > Eligibilities/Classifications
- Program Referrals > Program Referrals > Referral Information > Eligibilities/Classifications
- Family Cases > Case > Case Information > Eligibilities/Classifications
- Referral Application > Application Data for Providers > Application Information > Eligibilities/Classifications
- Go to: Client > Client Information > Personal Information > Eligibilities/Classifications.
- Search for and select a client.
- In the toolbar, select New Event and choose the eligibility event.
- In the Eligibilities / Classifications pane, complete the following required fields:
- Item - Select an item from the Program Eligibility dialog.
- Effective Date - (Required) - Select the calendar button and choose the date when the eligibility became effective.
- Complete any optional fields as appropriate, such as End Date, Rate, and Remarks.
- Select Save.
