Add Program Eligibility and Classifications

Use the Eligibilities/Classifications form in myEvolv to document eligibilities for the programs where a client, family case, or referral is enrolled.

Note: You can use the eligibilities/classifications as criteria for batch enrollment into a group.

  • Referral Application > Application Data for Providers > Application Information > Eligibilities/Classifications
  1. Go to: Client > Client Information > Personal Information > Eligibilities/Classifications.
  2. Search for and select a client.
  3. In the toolbar, select New Event and choose the eligibility event.
  4. In the Eligibilities / Classifications pane, complete the following required fields:
    • Item - Select an item from the Program Eligibility dialog.
    • Effective Date - (Required) - Select the calendar button and choose the date when the eligibility became effective.
  5. Complete any optional fields as appropriate, such as End DateRate, and Remarks.
  6. Select Save.
