Add Pregnancy History

Use the Pregnancy History form in myEvolv to record the history of pregnancies for a client. This form can be accessed for several different modules.

Menu paths

  • Client > Client Information > Health Information > Pregnancy History
  • Referral > Agency Referral > Health Information > Pregnancy History
  • Program Referrals > Program Referral > Health Information > Pregnancy History
  • Referral Application > Application Data for Providers > Health Information > Pregnancy History
  1. Go to one of the above listed modules, then search for and select a client or referral.
  2. In the toolbar, select New Event > Pregnancy History.
  3. In the Pregnancy History dialog, enter all relevant information. The Information Confirmed Date is a required field.
    Note: You can add information here about the number of previous pregnancies, deliveries, the type of birth, and whether the client obtained prenatal care.
  4. Select Save.
