General Information - Sites/Residentials
Here you can either create or edit your agency's Service Facilities. This will also include any sites that are noted as Managing Offices.

Enter General Information
- Navigate to the following path: Agency > Service Locations > Sites/Residentials > General Information.
- In the toolbar, click Select a Facility.
- Perform a search.
- If you are creating a new facility, click New. Enter information in the following fields as applicable:
- Name - Name to Identify the Facility
- Type - Type of Service Facility this location is. This field appears in different General Information forms for different types of locations. The list of Types is dependant on the type of Facility being set up.
- ID# - An ID number is used to make the facility unique. You can determine this internally or base the id on a license number. If you use more than one id, you can add a second id in the Other ID# field. For example, you may identify this facility as 11111, but it's certification # may be 22222. You can use this field and the Other ID# field to hold these values.
- Other ID# - An additional ID field if your facility uses more than one. If your facility utilizes the ElderMark HIE functionality, enter the Community ID supplied by HIE in this field.
Release Consideration: As of myEvolv versions 9.0.8185 and later, myEvolv supports ElderMark HIE.
- Tax ID, NPI#, and Default Taxonomy Code - These fields are used for 837 information.
- GL Facility Code (Enterprise Edition Only) - The GL code that identifies where clients are served or placed. (Foster homes will get theirs from the managing office associated to the home).
- Vendor for Payments (Enterprise Edition Only) - If you need to generate a payment to this facility in myEvolv, an entry must be made here.
- Region - This is a field that may use if you want to assign different geographical areas for your facilities (i.e. for reporting purposes). If this does not apply to your agency, simply select N/A from the pick-list.
- Address - The address of the Facility. Upon entering the zip code, the city and state will automatically be entered.
- Status History - Status of the facility. Notes whether the facility is Open/Reopened or Closed.
- Programs/Services Operated - Lists the Programs that operate through this facility.
Release Consideration: As of myEvolv versions 9.0.8185 and later, you can override the Program Unit and Cost Center for a program.
- Preferences - Typically used for Residential Facilities and Resource Families to note the type of clients they prefer to enroll.
- Additional Information - A remarks type field allowing you to add any additional information about the facility.
- Licensing Info - Enter specific Payer Specific licensing here.
- Staff - Lists staff members who are affiliated with this Facility.

For each program operated, enter in the Agency, Program, Program Unit.

Clients’ enrollments into a program require a facility to also be assigned. When you set up and define a facility, you also associate it with the programs that it operates. As a result, during the program enrollment process for a client the available listing of facilities is limited to those that have previously been associated to the corresponding program. In addition, when providing services for a client, users specify the program facility that is providing the service.

If you are editing an existing facility, click the facility name in the search results.

Once the form opens in the workspace, you can add or edit information.
Update the Managing Office Status
To declare a facility a managing facility, in Form Designer, you must make the fields listed below visible in the Service Facility form. By default, these fields are not available.
Complete these fields as appropriate to denote the managing facility status.
