Vocational Assessment

Record a client's problems as they relate to their ability to work, and their work history.

  1. Go to: Avatar CWS > Assessments > Product Assessments
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Select Ok.
  3. If the client has multiple episodes, select the client's episode, select Ok.
  4. In the Assessing Date field, enter the assessment date.
  5. In the Assessing Clinician field, enter the clinician's ID and click Process Search. Select the clinician, select Ok.
  6. In the Additional Dates field, enter the applicable dates. 
  7. In the Last School Attended field, enter in the applicable school name.
  8. In the Year Completed field, enter in an integer.
  9. In the Vocational/Work Related Training field, enter in the applicable information. 
  10. In the Client's Perception of How Illness Affects His/Her Ability to Work field, enter in the applicable information.
  11. In the Reason(s) Client Wants to Work field, enter in the applicable information.
  12. In the Physical or Mental Limitations on Ability to Work (Client's Perception) field, enter in the applicable information.
  13. In the Job Interests field, enter in the applicable information regarding job interests.
  14. In the Work Habits Client Perceives Need to Be Improved field, enter in the applicable information.
  15. In the Client's Job Related Goals field, enter in the applicable information.
  16. In the Services Client Sees as Necessary to Reach Goals field, select the applicable dictionary value(s).
  17. In the Expectations to Be Derived from Working field, enter in the applicable information.
  18. In the Current Work Skills and Potentials (Functional Capacities) field, enter in the applicable information.
  19. In the Functionality Limitations (Related Factors) field, enter in the applicable information.
  20. In the Personal Grooming and Appearance field, enter in the applicable information.
  21. In the Amenability to Vocational Counseling field, enter in the applicable information.
  22. In the Summary Pertinent Admission and Assessment Data field, enter in the applicable information.
  23. In the Conclusions and Recommendations field, enter in the applicable information.
  24. In the Problem field, enter in the applicable information.
  25. In the Goal field, enter in the applicable information.
  26. In the Objective field, enter in the applicable information.
  27. In the Intervention/Service field, enter in the applicable information.
  28. Navigate to the Past Work Record section on the left hand side. 
  29. Select Add New Item to add a row and enable the fields in the section.
  30. In the Employer field, enter in the applicable information.
  31. In the Address field, enter in the address.
  32. In the Job Title field, enter in the job title.
  33. In the Dates field, enter in the applicable dates.
  34. In the Weekly Wage field, enter in the applicable information. 
  35. In the Reason for Leaving field, enter in the applicable information. 
  36. In the Employer field, enter in the applicable information.    
  37. Once finished, click Submit.
    In the Past Work Record section, click Add New Item to add information for that section.
