Psycho-Social Assessment of Children

This assessment is used to record data on client psychosocial functioning, their environment, and legal issues. This assessment is specifically used for clients under the age of 18. 

  1. Go to: Avatar CWS > Assessments > Product Assessments
  2. In the Select Client screen, enter the client ID in the Client Name/ID field.
  3. If the client has more than one episode, select the appropriate episode. If the client only has one episode, this step will not be applicable. 
  4. The pre-display will only appear if a record has already been filed for that client and episode. Otherwise, the form will just open. If a pre-display does open, select Add. 
  5. The Client Name, Date of Birth, Age, Sex, Client Race, Client's Address - Street, Client's Address - City, Client's Address - State, and Client's Home Phone fields are disabled and auto filled with the data tied to their client profile. 
  6. In the Nickname field, enter the client's nickname. 
  7. In the Biological Parents field, enter the names of the client's biological parents. 
  8. In the Brothers and Sisters, with Age field, type the names of the client's siblings with age. 
  9. In the Persons Currently Living in the House field, enter the names of the people living in the house. 
  10. In the Who has legal custody of the child? field, enter the individual's name. 
  11. In the Any involvement of pt. with the Law? field, select Yes or No. If Yes is selected, enter a description in the Describe field. 
  12. In the Current school and grade field, type the client's current school and grade. 
  13. In the Is client in special class? field, select Yes or No. If Yes is selected, enter a description in the Describe field. 
  14. In the Schools attended since 1st grade field, enter the schools. 
  15. In the Repeated any grades? field, select Yes or No. If Yes is selected, enter a description in the Describe field. 
  16. In the Has received tutoring? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  17. In the Psychological testing in school? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  18. In the Speech therapy? field, select Yes or No.
  19. In the How is client doing academically? field, enter the response. 
  20. In the Behavioral problems? field, enter the client's behavioral problems. a
  21. In the Major problems in the family that might be affecting the child? field, enter the response. 
  22. In the How does client get along with other children? field, enter the response. 
  23. In the How does client interact with siblings? field, enter the response. 
  24. In the Did mother have normal pregnancy with this child? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  25. In the Was she taking meds? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  26. In the Other Substances? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  27. In the Did she have any medical or emotional problems? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  28. In the Was client normal at birth? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  29. In the Nutritional problems? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  30. In the Walked at field, enter the age the client began to walk. 
  31. In the Potty trained at field, enter the age the client was potty trained. 
  32. In the Anything that made client different from other children field, enter the response. 
  33. In the Are immunizations current? field, select Yes or No. 
  34. In the Is client on any medications? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  35. In the Prescribed by field, enter the response. 
  36. In the Does client see a doctor regularly field, select Yes or No. If Yes is selected, enter a response in the For what? field. 
  37. In the Has client been hospitalized since birth? field, enter a response. 
  38. In the Ever have an accident where client lost consciousness? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  39. In the Treated as an outpatient or in a hospital for emotional problems? field, select Yes or No. If Yes is selected, enter a description in the Describe field.
  40. In the What do you expect from treatment field, enter the response. 
  41. In the The above information has been given by field, enter the response. 
  42. In the Date field, select the date by typing it in MM/DD/YYYY format, using the calendar icon, or selecting the T or Y buttons for today or yesterday. 
  43. Navigate to the Presenting Problems section.
  44. In the Presenting Problems multi-iteration table, select Add New Item. 
  45. In the Presenting Problems field, enter the response.
  46. Add more presenting problems or navigate to the Identified Issues section. 
  47. In the Identified Issues multi-iteration table, select Add New Item. 
  48. In the Issues That Require Attention field, enter the response.
  49. Add more issues or navigate to the Plan section. 
  50. In the Plan multi-iteration table, select Add New Item. 
  51. In the Plan/Appropriate Referrals field, enter a response.
  52. Add more plans/referrals or navigate to the Medication Currently Used section. 
  53. In the Medication Currently Used multi-iteration table, select Add New Item. 
  54. In the Prescription Meds field, enter the medication. 
  55. In the Dosage field, enter the dosage information. 
  56. In the Frequency field, enter the frequency. 
  57. In the Over the Counter Meds field, enter over the counter medications. 
  58. In the Dosage field, enter the dosage information. 
  59. In the Frequency field, enter the frequency. 
  60. Select Add New Item to add more medication information, or ensure all required fields are filled out. 
  61. Once finished, click Submit.
