Psychological Evaluation
Record client problems that do not have a physical origin.
- Go to: Avatar CWS > Assessments > User Defined Assessments > Psychological Evaluation.
- Search for the Client Name/ID in Last Name, First Name format in the Select Client screen. Select the client name.
- If the client has multiple episodes, select the appropriate episode.
- If a record has been previously entered for the client, the pre-display will open. Select Add to add a new record.
- In the Assessing Clinician field, enter the clinician's name, click Process Search. Click Ok.
- In the Initial Evaluation Date field, enter the date.
- In the Additional Evaluation Dates field, enter the dates of any other evaluations.
- In the Referral Information field, enter applicable information.
- In the Background Information field, enter applicable information.
- In the Behavioral Observations (Clinical Observations) field, enter observations.
- In the Evaluation Information field, enter evaluation information.
- In the Interview Information field, enter interview information.
- In the Test(s) Administered field, enter information about the test(s) administered.
- In the Test Results field, enter information about the test results.
- In the Summary and Conclusions field, enter the summary and conclusion information.
- Navigate to the Diagnoses section.
- In the Axis I Primary field, enter a mental health or substance use disorder diagnosis and click the search button. Select an option.
- In the Other Conditions That may be a Focus of Clinical Attention field, enter conditions.
- In the Axis I Secondary field, enter a mental health or substance use disorder diagnosis and click the search button. Select an option.
- In the Other conditions that may be a focus of Clinical Attention field, enter other conditions.
- In the Axis I Tertiary field, enter a mental health or substance use disorder diagnosis and click the search button. Select an option.
- In the Other Conditions That may be a Focus of Clinical Attention field, enter other conditions.
- In the Axis II Primary field, enter a personality disorder and click the search button. Select an option.
- In the Traits field, enter information about the traits.
- In the Axis III Primary field, enter a general medical condition and click the search button. Select an option.
- In the Axis III Secondary field, enter a general medical condition and click the search button. Select an option.
- In the Axis III Tertiary field, enter a general medical condition and click the search button. Select an option.
- In the Axis IV field, select applicable problem(s) from the list.
- In the High GAF field, enter the highest GAF value.
- In the Low GAF field, enter the lowest GAF value.
- In the Current GAF field, enter the current GAF.
- In the Recommendations field, enter recommendation information.
- Click Submit.
