Psycho-Social Assessment

Record data on client psychosocial functioning, their environment, and legal issues.

Avatar CWS > Assessments > User Defined Assessments

1. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click Ok.
2. If the client has multiple episodes, select the client's episode, click Ok.
3. In the Date of Entry field, enter the assessment date.
4. Once finished, click Submit.

The following fields are set up in the RADplus Dictionary Update form (Avatar CWS):

• Substances Used
• Do You Have a History of the Following Behaviors
• Do You Have a Pending Case
