ProviderConnect Application Build

Details

Note: ProviderConnect modeling is compatible for forms in Avatar PM, and Avatar CWS.

• Forms must be modeled in Avatar PM (version 2006 and above) or Avatar CWS (version 2004 and above).

Steps

1. In the Select RADplus Forms field, select the forms data to send to ProviderConnect.
   Note: Forms with unsupported fields display an asterisk (*) next to the form title. The form data will be sent to ProviderConnect, data for unsupported fields will not be sent.

2. Click Submit.

• Modeled form data is compiled into XML code and sent to ProviderConnect.
• Depending on the amount of information being file, this process may take several minutes.

3. Use the Manage Dynamic Forms link to add the forms to the ProviderConnect menu.
