System Maintenance - Referral Source Maintenance - Cal-PM

Define the code for your referral source and manage referral source information. This information is used by the Admission Referral Information form.

  1. Go to: Avatar PM > System Maintenance > System Definition > Referral Source Maintenance
  2. In the Add Or Edit Referral Source field:
    • Select Add Referral Code.
    • Select Edit Referral Code.
  3. In the New Referral Source Code field, enter the code.
  4. In the Referral Source Code field, enter the code, click the Search button, and select the corresponding entry.
  5. In the Referral Source - Name field, enter the referral source name.
  6. In the Practitioner ID # field, enter the practitioner name, click the Search button, and select the corresponding entry.
  7. In the Referral Source - Specialty field, select the specialty.
  8. In the Referral Source Category field, select category.
  9. In the 'Referral Source' fields, enter referral source demographic information.
  10. Select Print Referral Source to generate a referral source report that lists referral source information entered in this form.
  11. Select File Referral.
