Admission Referral Information - Cal-PM

Enter a client’s referral source information. Editing information in this form does not affect the referral source.

• A referral source must be set up in the Referral Source Maintenance form.

1. Go to: Avatar PM > Client Management > Client Information
2. In the Select Client screen, enter the client ID in the Client Name/ID field, click the Search button, and select the corresponding entry.
   • The Episode Selection window shows if the client has multiple episodes.
   • Select an episode, click OK.
3. In the Primary Referral Source Code field, enter the referral source, click the Search button, and select the corresponding entry.
   • Searches can be performed by ID Number, Name, or Referral Source Category Code.
   • The Primary Referral Source - Category, and Specialty fields show the information defined in the Referral Source Maintenance form.
4. In the Demographic fields, enter referral source demographic information as appropriate.
5. When finished, click Submit.
