Scenario 1: Validate Upgrading Avatar SF 2023 to 2024 is successful when 2023.04.00 is loaded
Specific Setup:
  • Avatar SF 2023.04.00 is loaded.
Steps
  1. Open the "Product Updates" form.
  2. Select the appropriate [Namespace] from the Application dropdown list
  3. Click [Select Update/Customization Pack].
  4. Browse to the location for the updates and select the Update 1.
  5. Click [OK] on the "File Upload Complete" window.
  6. Click [Review Update/Customization Pack Contents].
  7. Verify Update 1 is included.
  8. Click [Install Update/Customization Pack].
  9. Click [OK] when the install completes.
  10. Click [Close Form].

Progress Note Correction - Note filed with the 'Clinical Quality Indicator
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:

Scenario 1: Progress Note Corrections - Validating reverted progress note for the new service with 'Clinical Quality Indicators'
Specific Setup:
  • Registry Setting:
  • The 'Enable VTSR' registry setting is set to 'Y'.
  • The 'Avatar CWS->System Maintenance->Progress Notes->->->Include Clinical Quality Indicators for Billing' registry setting is set to 'Y'.
  • Clinical Quality Code Configuration:
  • Desired CPT code is set up for the specific service code. Note the CPT code and service code.
  • Site Specific Section Modeling:
  • Progress Notes (Group and Individual) - Individual Progress Notes
  • Prompt Definition - Select Draft Note To Edit
  • Exclude from Data Collection Instrument- No
  • Admission:
  • An existing client is identified, or a new client is admitted. Note the client id, admission program, client name.
  • Practitioner:
  • An existing practitioner is identified to be used. Note the practitioner's id/name.
Steps
  1. Open the 'Progress Notes (Group and Individual)' form.
  2. Select desired client from the 'Select Client' field.
  3. Select 'New Service' from the 'Progress Note For' field.
  4. Select desired note type from the 'Note Type' field.
  5. Enter desired note in the 'Notes Field' area.
  6. Enter desired practitioner in the 'Practitioner'.
  7. Enter desired date in the 'Date Of Service' field.
  8. Select desired code from the 'Service Charge Code' field.
  9. Select desired entries from the 'Clinical Quality Indicators' field.
  10. Select 'Final' from the 'Draft/Final' field.
  11. Click [File Note].
  12. Validate the 'Progress Notes' dialogue contains 'Note filed'
  13. Click [No] on the 'Form Return' dialogue.
  14. Open the 'Crystal Report' or any other SQL data viewer tool.
  15. Locate to the CWS name space.
  16. Query the 'SELECT * FROM SYSTEM.cw_patient_notes'.
  17. Verify the 'date_of_service', 'PATID', 'clinical_quality_code', 'clinical_quality_value', 'draft_final_code' and 'draft_final_value' displays correct data.
  18. Close the report.
  19. Open the 'Progress Note Corrections' form.
  20. Select desired client from the 'Client ID' field.
  21. Select desired episode from the 'Episode Number' field.
  22. Click [Select Note to Correct].
  23. Select desired note from the 'Select Note To Correct' listbox.
  24. Click [OK].
  25. Select desired option from the 'Reason for Correction' dropdown list.
  26. Enter desired comments in the 'Comments' field.
  27. Select 'Revert Final Note to Draft' from the 'Correction Action' field.
  28. Select desired value from the 'Delete Service' field.
  29. Click [Submit].

  • Select State Form = New York State Client Admission Report (PAS 44N).
  • Select State Form Data Element = Program No. (10001).
  • State Form Text Value = desired value. Note the value.
  • Client:
  • A new client is admitted to the program identified above. Note client's id / admission date.
  • The diagnosis record is created for the client.
  • The 'Client Admission Report (PAS 44) has been filed.
  • The client has been discharged.
Steps
  1. Open the form 'Client Discharge Report (PAS 45N)'.
  2. Select desired client.
  3. Select the desired episode.
  4. Validate that the following fields have been added to the form: 'Was MAT Offered by this Program During this Treatment Episode?' and 'Was MAT Prescribed by this Program During this Treatment Episode?'.
  5. If 'No' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is disabled.
  6. If 'Yes' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is enabled and required,
  7. If 'No' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field defaults to 'None'.
  8. If 'Declined' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field defaults to 'None'.
  9. If 'Yes' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field will contain no predefined values and user can select items.
  10. Add data to all required fields and to desired optional fields.
  11. Submit the form.
  12. Close the form.
  13. Open the 'NYS PAS Form Submission File Creation' form.
  14. Populate the 'Provider ID No' with up to 5 digits.
  15. Set the 'Filter By PRU Number' field to 'No'.
  16. Select the month of discharge for the client identified above in the 'Month to report' field.
  17. Set the 'Re-Send Records Previously Sent' field to 'Yes'.
  18. Click [Create File].
  19. Verify the system displays all the records for the client for which the 'Client Discharge Report (PAS 45N)' form is filed.
  20. If desired, review the file created in the Facility Defaults output path and return to the form.
  21. Populate the 'Provider ID No' with up to 5 digits.
  22. Set the 'Filter By PRU Number' field to 'Yes'.
  23. 'Set the 'PRU Selected By' field to 'PRU'.
  24. Select the desired PRU value.
  25. Select the month of discharge for the client identified above in the 'Month to report' field.
  26. Set the 'Re-Send Records Previously Sent' field to 'Yes'.
  27. Click [Create File].
  28. Verify the system displays all the records for the client for the selected PRU number and for that program the 'Client Discharge Report (PAS 45N)' form is filed for the client.
  29. If desired, review the file created in the Facility Defaults output path and return to the form.
  30. Close the form.
Scenario 2: Crisis Episode Client Admission/Discharge Report (PAS 46N) - NYS PAS Form Submission File Creation
Specific Setup:
  • Facility Defaults:
  • The 'Output Path On Server For Electronic Files' field is set to the location where the output files are stored. Note the path.
  • State Form Dictionary Setup:
  • Set up desired programs.
  • Select State Form = New York State Client Admission Report (PAS 44N)
  • Select State Form Data Element = Program No. (10001)
  • State Form Text Value = desired value. Note the value.
  • Admission:
  • A new client is admitted in a program identified above.
  • Diagnosis:
  • The diagnosis record is created for the client.
  • Client Admission Report (PAS 44N):
  • Client admission report is filed for the clients admitted above.
  • Note the value of the ‘HIV Status Code’ field.
  • Discharge: Discharge the client.
  • Client Discharge Report (PAS 45N):
  • Client discharge report is filed for at least one client. Note the client id.
Steps
  1. Open the ‘Crisis Episode Client Admission/Discharge Report (PAS 46N)’ form.
  2. Enter the client in the ‘Select Client’ field matching the client used in the ‘Client Admission Report (PAS 44N)’ and 'Client Discharge Report (PAS 45N)' form.
  3. Select the same episode from the ‘Episode’ drop down list field as used in the ‘Client Admission Report (PAS 44N)’ form and 'Client Discharge Report (PAS 45N)' form.
  4. Verify that the value in the ‘HIV Status Code’ field is ‘Known to be Positive’ matches the value filed in the ‘Client Admission Report (PAS 44N)’ form.
  5. Populate all the required fields and any desired optional fields.
  6. Submit the form.
  7. Open the 'NYS PAS Form Submission File Creation' form.
  8. Populate the 'Provider ID No' with up to 5 digits.
  9. Set the 'Filter By PRU Number' field to 'No'.
  10. Select the month of admission for the clients identified above in the 'Month to report' field.
  11. Set the 'Re-Send Records Previously Sent' field to 'Yes'.
  12. Click [Create File].
  13. Verify the system displays all the records for the client for which the 'Client Admission Report (PAS 44N)', 'Client Discharge Report (PAS 45N)' and 'Crisis Episode Client Admission/Discharge Report (PAS 46N)' forms are filed.
  14. Set the 'Filter By PRU Number' field to 'Yes'.
  15. Set the 'PRU Selected By' field to 'PRU'.
  16. Select desired 'PRU'.
  17. Select the month of admission for the clients identified above in the 'Month to report' field.
  18. Set the 'Re-Send Records Previously Sent' field to 'Yes'.
  19. Click [Create File].
  20. Verify the system displays all the records for the client for the selected PRU number.
  21. Populate the 'Provider ID No' with up to 5 digits.
  22. Set the 'Filter By PRU Number' field to 'Yes'.
  23. Set the 'PRU Selected By' field to 'Program'.
  24. Select one of the programs identified in the setup section.
  25. Select the month of admission for the clients identified above in the 'Month to report' field.
  26. Set the 'Re-Send Records Previously Sent' field to 'Yes'.
  27. Click [Create File].
  28. Verify the system displays all the records for the client for the selected Program.
  29. Close the form.

  4. Validate that the following fields have been removed from the form: 'NYSID', 'CJ Consent Date', and 'CJ Consent Revoke Date'.
  5. Validate that the following additional fields have been added to the form: 'Transgender', 'Asian Origin', and 'Pacific Origin'.
  6. Validate that the following fields have been added to the form: 'Was MAT Offered by this Program During this Treatment Episode?' and 'Was MAT Prescribed by this Program During this Treatment Episode?'.
  7. If 'No' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is disabled.

Specific Setup:
  • Guarantors/Payors:
  • Three existing guarantors are identified to be used. One of them is with a financial class of 'Self-Pay'. Note the guarantor code/name.
  • Service codes:
  • An existing service code is identified to be used. Note the service code/description.
  • Service Fee/ Cross Reference Maintenance:
  • A fee definition is created for the service code identified in the ' Service Codes' form using the revenue code. Note the revenue codes for the service code(s) identified.
  • Admission:
  • An existing client is identified or a new client is admitted. Note client id, admission program, admission date.
  • Financial Eligibility:
  • The guarantors identified in the 'Guarantors/Payors' form is assigned to the client as a primary, Secondary and Self-Pay as a tertiary guarantor.
  • A financial eligibility record for the client is created such that the liability distributed across all 3 guarantors.
  • Recurring Client Charge Input:
  • 5-7 services are rendered to the client. Note service dates, service codes.
  • Client Ledger:
  • The services are distributed correctly to the assigned guarantors.
  • Dictionary Update:
  • File - Payor
  • Data Element - Financial Class
  • Dictionary Code - Desired code for the financial class of the third party guarantor
  • Dictionary Value - Financial class of the third party guarantor

Steps
  1. Open the 'Michigan County Bill' form.
  2. Select "Compile" from the 'Compile, Print, or Delete' field.
  3. Set the 'Print Charges Thru' field to a desired date.
  4. Select "No" in the 'Create Claims' field.
  5. Select "No" in the 'Print For Interim Batch' field.
  6. Select desired guarantors from the 'Guarantor(s)' field.
  7. Select "Individual" from the 'Print For An Individual Or All Clients' field.
  8. Select desired client from 'Client ID' field.
  9. Click [Process County Bill].
  10. Validate the bill compiles successfully.
  11. Validate the crystal report launched successfully and includes following details:
  12. Validate 'DISCHARGE DATE' field exist under 'CUR ADMIT DATE' field.
  13. Validate 'DISCHARGE DATE' field displays the date of discharge when the client is discharged from the episode.
  14. Validate 'DISCHARGE DATE' field does not display any value when the client is active in the episode.
  15. Validate the total charges in the 'PATIENT MONTHLY TOTAL' section between days and liability.
  16. Validate the total charges for each patient for each month grouped by revenue code.
  17. Validate the guarantor name and payment amount for all third party guarantors other than billing guarantor.
  18. Validate the total billed to self-pay guarantor.
  19. Close the report.
  20. Close the form.
  21. Open the 'Crystal Report' or any other SQL data viewer.
  22. Run the SQL query to retrieve data from the 'STATEFORM.mich_county_bill_census' table.
  23. Validate the 'discharge_date' field is added in the 'STATEFORM.mich_county_bill_census' table.
  24. Run the SQL query to retrieve data from the 'STATEFORM.mich_county_bill_stmt' table.
  25. Validate the 'total_charges' field is added to the table and contains correct charges.
  26. Validate the 'total_billed_self_pay' is added to the table and contains correct charges distributed to the self pay gaarantor.
  27. Validate the 'third_party_guar_id' field is added to the table and displays correct guarantor code for the third party guarantor.
  28. Run the SQL query to retrieve data from the 'STATEFORM.mich_county_bill_detail' table.
  29. Validate the 'total_charges' field is added to the table and contains correct charges.
  30. Validate the 'total_billed_self_pay' is added to the table and contains correct charges distributed to the self pay gaarantor.

Scenario 2: Michigan County Bill - Active Outpatient client
Specific Setup:
  • Guarantors/Payors:
  • Three existing guarantors are identified to be used. One of them is with a financial class of 'Self-Pay'. Note the guarantor code/name.
  • Service codes:
  • An existing service code is identified to be used. Note the service code/description.
  • Service Fee/ Cross Reference Maintenance:
  • A fee definition is created for the service code identified in the ' Service Codes' form using the revenue code. Note the revenue codes for the service codes identified.
  • Admission (Outpatient):
  • An existing client is identified or a new client is admitted. Note client id, admission program, admission date.
  • Financial Eligibility:
  • The guarantors identified in the 'Guarantors/Payors' form is assigned to the client as a primary, Secondary and Self-Pay as a tertiary guarantor.
  • A financial eligibility record for the client is created such that the liability distributed across all 3 guarantors.
  • Diagnosis:
  • The diagnosis record is created for the client. Not the diagnosis date. Client should have active diagnosis record at the date of service.
  • Recurring Client Charge Input:
  • 5-7 services are rendered to the client. Note service dates, service codes.
  • Client Ledger:
  • The services are distributed correctly to the assigned guarantors.
  • Dictionary Update:
  • File - Payor
  • Data Element - Financial Class
  • Dictionary Code - Desired code for the financial class of the third party guarantor
  • Dictionary Value - Financial class of the third party guarantor
  • Extended Dictionary Data Element - Exclude from Michigan County Bill as Third Party
