Product Update Description

Avatar CareFabric is enhanced to include the capability of saving signature fields received from CareFabric Assessments into user-modeled forms. CareFabric Assessments can be mapped to the user-modeled forms utilizing the 'CareFabric Assessment Mapping' form.

CareFabric Assessment Mapping - Support for Signature fields
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • CareFabric Assessment Management
  • CareFabric Assessment Mapping
  • Consumer Portal
Scenario 1: CareFabric Assessment Mapping - Validate assessments with signatures from myHealthPointe
Specific Setup:
  • Avatar CareFabric must be configured to integrate with myHealthPointe.
  • A modeled form must be defined in Avatar with the following (Avatar Form):
  • 'Text' field
  • 'Signature' field
  • 'Draft/Final' field
  • A form must be defined in myHealthPointe with the following (MHP Form):
  • 'Text' field
  • 'Signature' field
  • The details from the "MHP Form" must have been sent to Avatar from myHealthPointe.
  • In 'CareFabric Assessment Management', "Yes" must be selected in the 'Save Draft Documents' field for the "MHP Form".
  • A client is enrolled in an existing episode with the following (Client A):
  • 'Date of Birth' on file
  • 'Email Address' on file
  • Login credentials for myHealthPointe
  • "MHP Form" awaiting completion in myHealthPointe
Steps
  1. Access the 'CareFabric Assessment Mapping' form.
  2. Select "MHP Form" in the 'Select Assessment' field.
  3. Select "Avatar Form" in the 'Selected Modeled Form' field.
  4. Click [Launch Mapping Grid].
  5. Validate the 'CareFabric Assessment Mapping' grid is displayed.
  6. Click [New Row].
  7. Select the 'Text' field in the 'Product/Modeled Form Field' field.
  8. Select the 'Text' field in the 'Assessment Field' field.
  9. Click [New Row].
  10. Select the 'Signature' field in the 'Product/Modeled Form Field' field.
  11. Select the 'Signature' field in the 'Assessment Field' field.
  12. Click [Save], [Yes], and [File Mappings].
  13. Close the form.
  14. Log in to the myHealthPointe Consumer Portal for "Client A".
  15. Navigate to the 'Documents Awaiting Completion'.
  16. For the "MHP Form", click [Review and Complete].
  17. Enter the desired value in the 'Text' field.
  18. Sign in the 'Signature' field.
  19. Complete the form.
  20. Please note: the form may take a few minutes to send to Avatar, it is not instantaneous.
  21. Select "Client A" and access the "Avatar Form".
  22. Validate the record sent from myHealthPointe is displayed.
  23. Click [Edit].
  24. Validate the 'Text' filed in myHealthPointe displays as expected.
  25. Validate the 'Signature' filed in myHealthPointe displays as expected.
  26. Close the form.
