Using EHI Export

Use the EHI Export functionality in myAvatar for export of data for a single patient or the entire population.

Go to:

  1. Search for the EHI Export form or go to Avatar PM > RADplus Utilities > EHI Export.
  2. In the Define EHI section, complete the following:
    • Select Forms Included in EHI Export. 
      Note: Document image paths (i.e., scanned and imported images) may be included in the EHI Export by including Avatar PM > Documents in the forms export list.
    • Select the Integrated Application Assessments to include in EHI Export.
  3. Select Save to save the configuration. 
    Note: This applies to current export and will be available for the next export.
  4. In the Export EHI section of the form and complete the following:
  5. Select Data and Documentation, Data Only, or Documentation Only for Export the Following.
  6. Select the Client (if for a single client). 
    • If not for a single client, leave blank to export all.
  7. To limit output by data entry date, select the Entered During Start Date and Entered During End Date in MM/DD/YYYY format.
  8. The Select Export Location is a defaulted field that provides the SFTP server file location target for the export.
  9. Enter the Reason for Export to capture why the export is being processed.
  10. Select Process to create the export.
