Intake workflow

With the Intake workflow in myAvatar NX you can:

  • Manage inbound referrals and send external referrals from the CareConnect Inbox, available from the summary on the myDay View.
  • View a client's CCD (Continuity of Care) document, with all the clinical/demographic information included. 
  • Manage the referral electronically. (When you accept or reject the referral, the sender will receive an electronic response.) 
  • Launch the Admission bundle to admit a new client or an existing client.

Admission bundle

The Admission bundle is a collection of Admission forms configured by your organization for processing admissions. 

If this is a new client, click Admit New Client to open the Admissions Bundle (the collection of admission forms configured for your organization). This will pull over any applicable data from the CCD and auto populate the form with the demographic information. 

If this is an existing client, you can admit them and, if any of the information is different in the CCD, the admission forms will update. 

As you complete and submit each form in the Admission bundle, the next form opens. 

After all forms in the bundle have been completed, navigate to the scheduling calendar. 

Schedule appointments

  • Use the filtering options to the right of the scheduling calendar to narrow the providers seen on the calendar that can have appointments scheduled.
  • Use the links to forms to find existing appointments (If a client cannot remember when their appointment is you could look here) and also find open appointment times (This is helpful if there is a client with very specific requirements for whom they want to see.)
  • On the calendar the gray areas are blocked off time where nothing can be scheduled. 
  • To reschedule an appointment, either drag and drop the appointment or right-click the appointment and reschedule.
