Consent For Access - Referral Section - Cal-PM

Grant or revoke consent for sending and receiving referrals.

A referral consent must be created for the client in this form in order to create a referral in the New Referrals form.


  1. Go to: Avatar PM > Client Management > Client Information > HIE Section
  2. The Consent for Referral grid displays the information for the system's current referral consents.
    • Once filed, individual rows can be edited but not deleted.
  3. In the Providers field, select the providers to grant consent. This field is populated through CareConnect.
    Note: The Referral Management Configuration form must be filled out.
  4. In the Consent to Send field:
    • Select Grant if the client has granted consent for the organization to send referrals to the specified provider.
    • Select Revoke if the client has revoked consent for the organization to send referrals to the specified provider.
  5. In the Start and End Date fields, enter the beginning and ending dates for the consent, as required.
    • If consent is being revoked, the End Date will not be enabled.
  6. Select Submit.
