Admission Assessment (SW)

  1. Go to: Avatar CWS > Avatar CWS Assessments > User Defined Assessments
  2. Select the correct client and episode.
  3. In the Assessment Date field, enter the assessment date.
  4. In the DCM/ICM field, select whether or not the patient has either idiopathic dilated cardiomyopathy (DCM) or ischemic cardiomyopathy (ICM).
  5. In the Contact and Phone # fields, enter the names and numbers of the client's contacts.
  6. In the Income/Financial Supports field, select all sources of income that apply.
  7. In the Evaluation of Needs field, enter any comments on the client's needs.
  8. In the Current living arrangements/support system field, enter any relevant information about the client's current living situation.
  9. Once finished, click Submit.
