View Demographics
Patient demographics entered through myAvatar can be viewed in both the Provider Portal and the Consumer Portal of myHealthPointe.
Prerequisite:
- Patient demographics have been entered via the Admission or Update Client Data forms in myAvatar.
- The Client Name field in the myAvatar form is in the format of Last,First (no space).
- The Email Address field in the myAvatar form is required for the patient to be credentialed in myHealthPointe.
- The Consent On File For Use of Integrated eSignature must be enabled/checked in the myAvatar form for Document Routing.
- The Cell Phone field must be completed if the client wants to receive notifications via text, but it is not required for myHealthPointe.
Note: The information displaying under Demographics in myHealthPointe is pulled from myAvatar. Updating the form in myAvatar will update the fields in myHealthPointe.
