Follow-Up - STD module
In the STD module, the Follow-Up form is where you record the post-discharge actions you've taken regarding the patient's positive test results and the actions the patient is to take post-discharge. Also, this form has the option to lock or unlock an encounter to control whether the encounter information can be edited.
On the main menu, navigate to Post-Discharge > Follow-Up.
- The Encounter Locking feature lets you lock an encounter post-discharge when the chart is considered complete for the encounter.
- All staff members are capable of locking and unlocking encounters.
Positive result follow up - Initial actions
- Under the Positive Result Follow Up - Initial Actions grid, click Add Entry.
- In the Add Positive Result Follow Up window, in the Test Result box click the arrow and select the result.
- In the Presumptively Tested box, click the arrow and select Yes or No.
- Continue with entering information for each row as appropriate.
- When finished, click Save Entry.
Informed
- In the Informed row, click the arrow
.
- In the Informed and Informed By boxes, click the arrow and select the appropriate choice.
- For the Informed Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Informed Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
First, second, and third call
- In the row, click the arrow
.
- In the Call and Call By boxes, click the arrow and select the appropriate choice.
- For the Call Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Call Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
Letter sent
- In the Letter Sent row, click the arrow
.
- In the Letter Sent and Third Call By boxes, click the arrow and select the appropriate choice.
- For the Letter Sent Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Letter Sent Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
Positive result follow up - CDI referral and treatment
- Under the Positive Result Follow Up - CDI Referral and Treatment grid, click Add Entry.
- In the Add Positive Result Follow Up window, in the Test Result box click the arrow and select the result.
- Continue with entering information for each row as appropriate.
- When finished, click Save Entry.
Referred to CDI
- In the Referred to CDI row, click the arrow
.
- In the Referred to CDI and Referred to CDI By boxes, click the arrow and select the appropriate choice.
- For the Referred to CDI Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Referred to CDI Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
FR Completed
- In the FR Completed row, click the arrow
.
- In the FR Completed and FR Completed By boxes, click the arrow and select the appropriate choice.
- For the FR Completed Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the FR Completed Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
Treated
- In the Treated row, click the arrow
.
- In the Treated and Treated By boxes, click the arrow and select the appropriate choice.
- For the Treated Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Treated Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
Field treatment
- In the Field Treatment row, click the arrow
.
- In the Field Treatment box, click the arrow and select Yes or No.
- In the Field Treatment From Scanned box, click the arrow and select the appropriate choice.
- For the Field Treatment By Date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Field Treated Time, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
- Click Done.
At the bottom of the form:
- For the Patient will return to clinic for treatment on date, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the following items, select Yes if applicable.
- Advised to notify all partners within the last 60 days
- Advised to return to clinic for recheck in 3 months
- For the Date Advised, enter the date. Optionally click the arrows or the calendar
to select a date.
- For the Time Advised, click the arrows to select the hour, minute, and AM/PM, or click the clock
to select a time.
Encounter locking
The encounter locking functionality prevents other users who may be on the encounter from entering data for the encounter. The ability to toggle between locking and unlocking an encounter is available in case a modification must be made after an encounter was locked. The Lock Encounter History grid shows a running history of who locked/unlocked the encounter and when the action occurred.
- For the Lock encounter from future edits item, select Yes to prevent other users from making future edits to the encounter. An entry for the locked encounter appears in the Lock Encounter History grid.
- On the toolbar above the menu, click the Save icon
