Diagram for Lesions - STD module

In the STD module, the Diagram form is where you document your observations of the patient's lesions. The lesion information is view-only unless you are on the current encounter. 

On the main menu navigate to Physical Exam > Diagram.

View lesion information

  1. In the Encounter box, click the arrow and select an encounter. This field defaults to the encounter currently selected on the Encounter form.
  2. Review the diagrams and note the progression of the lesions from one encounter to another.

Document lesions

You can only enter lesion information if you are on the current encounter. 

  1. In the Encounter box, click the arrow and select an encounter. This field defaults to the encounter currently selected on the Encounter form.
  2. The Observed By field defaults to the staff member who is logged in and cannot be modified. 
  3. On the appropriate diagram, click the location(s) of the lesion(s). A red dot appears for each lesion.
  4. To remove a lesion from a diagram, click the red dot to select it, then click Delete Lesion above the diagram.
  5. In the Description box, enter a description of the patient's lesion(s).
  6. On the toolbar above the menu, click the Save icon .
