TEDS Discharge

Discharge a client from a TEDS admission program.

Feature

The Treatment Episode Data Set (TEDS) provides information about client admissions and discharges to substance abuse treatment programs. 

  • TEDS discharges can be entered and edited in ProviderConnect, and in the TEDS Discharge form (Avatar PM).

  • TEDS information entered in ProviderConnect is sent to Avatar PM, and from Avatar PM to the Michigan state agency for reporting.

  • TEDS discharges filed in ProviderConnect have a transaction status of Pending (Adm. Status column). The transaction status is updated when the TEDS discharge record is received in Avatar PM.

  1. In the Main Menu click Lookup Client.

  • The Search Criteria screen displays.
  1. Enter the client's name, ID, social security number.

  2. Click Search By Criteria.

  • The Search Results screen displays.
  1. Click a client ID number.

  • The ProviderConnect Demographic screen displays for the client and agency.
  1. In the Client Sidebar menu click TEDS.

  • The TEDS Admissions & Discharges screen displays.
  1. Click the Click to Discharge link for an  admission date (Discharge Date column).

  • The Discharge Form screen displays.

Create a TEDS Discharge

  1. Enter the client's discharge date and time.

  • Enter dates in the format m/d/yyyy.
  1. Select the service category.

  • If the service category selected is Residential Detox, a Detox Aftercare Plan appointment must have been filed for the client at least 90 days before the TEDS discharge.
  1. Select the client's primary reason for discharge.

  2. Select the client's referral to other area.

  3. Enter the total number of client:
    Arrests.
    Possessions.
    Automobile drug convictions.

  4. Select the client's corrections related status.

  5. Select the client's substance usage in the past 30 days.

  6. Select the frequency of client attendance in self-help programs in the last 30 days.

  7. Click Save Discharge.

  • The TEDS Admission & Discharges screen displays.
  • The client's discharge date displays in the Discharge column.
  • The discharge reason displays in the Dis. Reason column.
  • The discharge status displays in the Dis. Status column.

Generate the Audit Log Report to view information traffic sent from ProviderConnect to Avatar PM.
