TEDS Admission

Enter a client into a TEDS treatment program in ProviderConnect.

The Treatment Episode Data Set (TEDS) provides information about client admissions and discharges to substance abuse treatment programs. 

  • TEDS admissions can be entered and edited in ProviderConnect and in the TEDS Admission form (myAvatar PM).
  • TEDS information entered in ProviderConnect is sent to myAvatar PM, and from myAvatar PM to the state agency for reporting.
  • TEDS admissions filed in ProviderConnect have a transaction status of Pending (Adm. Status column). The transaction status is updated when the TEDS admission record is received in myAvatar PM.
  • The client's date of birth must have been filed (Admission or Update Client Data form, myAvatar PM).
  • An PI admission and PI assessment appointment record must have been filed for the client 90 days before the client's TEDS admission date.
  • The provider must be set to perform TEDS reporting (TEDS Provider Setup form, myAvatar MSO).

Note: After a TEDS admission is submitted it cannot be edited.


  1. In the Main Menu select Lookup Client.
    Result: The Search Criteria screen displays.
  2. Enter the client's name, ID, and Social Security number.
  3. Select Search By Criteria.
  4. In the Search results, select a client ID number.
    Result: The ProviderConnect Demographic screen displays for the client and agency.
  5. In the Client Sidebar menu select TEDS.
  6. In the TEDS Admissions & Discharges screen select Add Admission Record.
    Result: The Create Admission screen displays.
  7. In the Admission Date field, enter the date, select Submit.
    Note: Enter dates in the format m/d/yyyy.
    Result: The Admission screen displays.
  8. Select Save Admission.
    Result: The TEDS Admissions & Discharges screen displays.

Generate the Audit Log Report to view records sent between ProviderConnect and Avatar MSO.
