PI Sentinel Event

PI sentinel event records display information filed in ProviderConnect, and in the PI Sentinel Event Form form (Avatar MSO).

Details

A sentinel event is an unanticipated event resulting in a client's death or injury, and is unrelated to the natural course of the patient's illness. Performance indicator (PI) sentinel event records entered in Avatar MSO are sent to ProviderConnect.

PI sentinel event records:

  • can be entered and edited in either ProviderConnect or in the PI Sentinel Event Form form (Avatar MSO).

  • filed in ProviderConnect are sent to Avatar MSO, and from Avatar MSO to the Michigan state agency.

  1. In the Main Menu click Lookup Client.

  • The Search Criteria screen displays.
  1. Enter the client's name, ID, social security number.

  2. Click Search By Criteria.

  • The Search Results screen displays.
  1. Click a client ID number.

  • The ProviderConnect Demographic screen displays for the client and agency.
  1. In the Client Sidebar menu click Appointments.

  • The Appointments screen displays.

Create a PI Sentinel Event

  1. In the Sentinel Events section, click Add Sentinel Event.

  • The Sentinel Event screen displays.
  1. Enter the first assessment request date in the format m/d/yyyy.

  2. Select the sentinel event type.

  3. Enter the sentinel event date.

  4. Enter comments associated with the sentinel event.

  5. Select whether the event was investigated.

  • If Yes, enter the investigation date, and enter investigation comments.
  1. Select whether action was taken after the investigation.

  • If Yes, enter the date action was taken, and enter comments.
  1. Click Save Record.

Edit a PI Sentinel Event

  1. In the Sentinel Event section, click an event type.

  2. Edit selections as appropriate, click Save Record.

Generate the Audit Log Report to view records sent between ProviderConnect and Avatar MSO.
