Create A Provider Admission

Create a provider admission.

Details

  • The Provider Admission link in ProviderConnect has the same functionality as the Admission form in Avatar PM.
  • ProviderConnect coordinating providers can create outpatient provider admissions.
  • Avatar Cal-PM - Once the admission date and program are selected, the Admitting Practitioner and Attending Practitioner fields are populated by all Avatar Cal-PM practitioners allowable for the admission date and are not limited by program.
  • The user must have been assigned to an access group with 'Create Provider Admission' permission (ProviderConnect Manage Permissions link, Manage an Access Group).

  • In order to view provider admissions, the user must be assigned to an access group with 'View Provider Admission' permission.

  • In order to view multiple provider admissions, the user must be assigned to an access group with 'Create Multiple Provider Admissions' permission.

  1. In the Main Menu click Add New Client/Client Search.

  • The Search Criteria screen displays.
  1. Enter the client's last name and their social security number, in the format ###-##-####.

  2. Click Search by Criteria.

  • The client is searched in ProviderConnect.
  • If the client is not found, the client is searched in Avatar PM.
  • If the client is not found, a new client can be added to ProviderConnect.
  1. Click Create New Admission.

  • The Admission Information screen displays.
  • If applicable, enter admission comments.

  • If applicable, enter the admission department time out in the format HH:MM AM/PM.

  • Click Save Admission to file the admission record.

  • The admission can be reviewed through the Provider Admission link.

  • Clients are assigned a temporary ProviderConnect ID in the format TMP ##. The temporary client ID is replaced by a permanent ID when admission data is received by Avatar PM.
