Create a provider diagnosis.
Details
- The Provider Diagnosis link in ProviderConnect provides the same functionality as the Diagnosis form in Avatar PM.
- The Diagnosis form uses the ICD-9 format. See Create a Provider Diagnosis (ICD-10) for information about the Diagnosis form in ICD-10 format.
- ProviderConnect coordinating providers can create provider diagnoses.
- In the Diagnosis pre-display, users can view/access Diagnosis records only for associated Admission episodes under Avatar PM programs mapped to the user's contracting provider via the Avatar MSO, MSO to Parent System Integration Mapping form). Also within the Diagnosis form, the episode selection field is limited to admission episodes that are under Avatar PM programs mapped to the user's contracting provider.

The user must have been assigned to an access group with Coordinating Provider permission (ProviderConnect Manage Permissions link).

The user must belong to an agency where the client has a coordinating provider assignment valid for the current date.

In the Main Menu click Lookup Client.
- The Search Criteria screen displays.

Enter the client's name, ID, social security number.
- To display all clients, enter no data, click Search by Criteria.

Click Search By Criteria.
- The Search Results screen displays.

Click a client ID number.
- The ProviderConnect Demographic screen displays for the client and agency.

In the Client Sidebar menu click Provider Diagnosis.
- The Provider Diagnosis screen displays.

Click Add Diagnosis Record.
- The Diagnosis Information screen displays.

Click Save Diagnosis to file the information.
- The Provider Diagnosis screen displays.
- Clicking a diagnosis date displays the client diagnosis record information in display only.
