Medical Diagnosis (Alternate) - PM

Input Medical Diagnosis codes for a client on a specific diagnosis date.

Note: As of myAvatar PM 2014 Update 191, records filed with this form will not convert to the new Diagnosis form upon updating to ICD-10.

When the Enable Acute Billing and the Use Medical Diagnosis Option For Medical Diagnosis Bills (page A-65) registry settings are enabled, the electronic 837I bill will use diagnosis codes in the following priority sequence:

  1. Medical Diagnosis codes entered in the Client Charge Input With Diagnosis Entry form.
  2. The Medical Diagnosis (Alternate) form.

Prerequisites:

  • This form is available when the Enable Acute Billing registry setting is enabled.
  • Procedure codes must be defined in the ICD-9 Procedure Code Definition form.
  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Client screen, enter the client name or ID, and select. Click Select.
  3. In the Type of Medical Diagnosis field, select whether the diagnosis is an admitting diagnosis, a discharge diagnosis, or an update to an existing diagnosis.
  4. In the Date of Medical Diagnosis field, enter the date for the diagnosis.
  5. In the Time of the Medical Diagnosis field, enter the time for the diagnosis.
  6. For the Medical Diagnosis - 1 field, enter all or part of an ICD-9 code or a diagnosis name and select the diagnosis from the list.
  7. In the Medical Diagnosis - 2 through Medical Diagnosis - 30 fields, enter additional medical diagnoses as appropriate.
  8. For the Principal Medical Diagnosis field, enter the diagnosis.  
  9. For the Medical Diagnosis Practitioner field, enter the practitioner name, and select.
  10. When finished, click Submit.
