Care Notes and Documentation

Document correspondence with a member, or about a member. This is a user-defined form.

  1. In the Select Client screen, search for and select the client by name or ID.

  2. In the Contact Date field, enter the date of the contact.

  3. In the Start Time field, enter the start time of the contact.

  4. In the End Time field, enter the time at contact closure.

  5. In the Contact Type field, select the contact.

  6. In the Contact Initiated By field, select the contact.

  7. In the Mode of Contact field, select Phone or Mail/Fax/E-mail as the method of contact.

  8. In the Provider / Facility field, enter the provider name, and select.

  9. In the Contract field, enter the contract funding source.

  10. In the Other Representative field, enter the member representative.

  11. In the Appeal Type field:

  • Select Claim if the claim is being appealed.

  • Select Authorization if the authorization is being appealed.

  1. In the Contact Documentation field, enter associated notes.

  2. Click View Contact Notes For This Member to generate a report that contains all contact types, and notes for the member.
    Each row in the report represents a single contact event.

  3. Click View Notes to display contact notes.

  4. When finished, click Submit.
