Enter care criteria information.

Depending on the selection in the Contact Type field (Contact Notes / Documentation section), specific fields will be enabled on this section.

  1. Enter member care information as appropriate.

  2. Click View/Print Assessment Scores to generate the Functional Assessment report that details the member's functional assessment records (Functional Assessment form).
